Single stage local flap, simple and small. Mustnít be in association with any items between 3 1 3 5 6 and 3 1 3 7 6.

 

Item 4 5 2 0 1

Muscle, myocutaneous or skin flap, where clinically indicated to repair one surgical excision made in the removal of a malignant or non malignant skin lesion. Must be in association with 3 1 0 0 0 to 3 1 0 0 5, 3 1 3 5 8 to 3 1 3 6 0, 3 1 3 6 3 to 3 1 3 6 4, 3 1 3 6 9 to 3 1 3 7 1, 3 1 3 7 3 or 3 1 3 7 6. Claimed once per defect.

 

Item 4 5 2 0 2

Muscle, myocutaneous or skin flap, to repair one surgical excision made in the removal of a malignant or non malignant skin lesion, if patientís record is clearly annotated with clinical relevance of the procedure and either,

 

item 4 5 2 0 1, applies and additional flap repair is required for the same defect; or

item 4 5 2 0 1, doesnít apply, for example the excision isnít one of the associated items, and either

The patient has severe pre-existing scarring, severe skin atrophy or sclerodermoid changes, or the repair is contiguous with a free margin.

Item 4 5 2 0 3

Single stage local flap, complicated or large, excluding flap for male pattern baldness and H-flap or double advancement flap. Mustnít be in association with any items between 3 1 3 5 6 and 3 1 3 7 6.

 

Item 4 5 2 0 6

Single stage local flap, on eyelid, nose, lip, ear, neck, hand, thumb, finger or genitals, excluding H flap or double advancement flap. Mustnít be in association with any items between 3 1 3 5 6 and 3 1 3 7 6.

 

Item 4 5 2 0 7

H flap or double advancement flap if indicated to repair one defect, on eyelid, eyebrow or forehead. Mustnít be in association with any items between 3 1 3 5 6 and 3 1 3 7 6.
